A maintenance service report records the asset, the reason for the visit, findings, work performed, checks completed and follow-up. It allows the customer and the next technician to distinguish completed work from unresolved issues and tasks requiring another visit. A useful report preserves those distinctions even when the original problem has not been resolved.

What should a maintenance service report contain?

The diagnosis section may include a hypothesis if it is identified as one. “Fault may be related to a connection” is different from “faulty connection confirmed.” If the cause has not been established, state that plainly.

Add the fields required for your trade. IT maintenance, building services and machinery maintenance involve different checks. This template documents the work; it does not prescribe the technical operations to perform.

Worked example: intermittent printer failure

This example is fictional. References and results are included only to demonstrate clear reporting.

The result is specific. Three successful tests have not been turned into a guarantee that the fault will never return. The next technician knows what changed and what to ask the customer.

How to dictate an accurate service recap

Start with the asset identifier. Separate what the customer reported from what you observed. List completed operations before discussing proposed work. Finish with the checks and follow-up.

A useful spoken recap would be: “Job INT-024, printer IMP-04 at reception. Customer reports interruptions. I observed one. Network cable replaced. Three test documents printed without error. Full-day operation still needs observation. Jules calls Nora on 10 September.”

Pay particular attention to numbers, units, references and negative statements during review. “Part not replaced” must not become “part replaced.” The voice note to report method includes a prompt that preserves missing and uncertain information.

Prepare the next visit

An incomplete handoff makes the next technician repeat the original questions. Record the open issue, checks already performed, required part or access, and the contact who can help unblock the work.

Separate a request for a part from a confirmed order. If delivery determines when work can resume, record the dependency and assign someone to check availability. A return date given without that context can become a commitment the team cannot meet.

Technical report and customer update

The technical report contains what is needed to continue the work. A customer update can be shorter: the issue addressed, observed result, remaining limitation and next contact. The two versions should agree.

For example: “The network cable has been replaced. The on-site tests did not reproduce the interruption. We will contact you tomorrow to check performance during normal use.” This is precise without exposing every diagnostic note.

Practical questions

Is a work order the same as a service report?

Terminology varies. A work order may define and assign the job; a service report describes what happened. Check that the final record distinguishes the original request, work completed and observed outcome, whatever your organization calls it.

What if no repair was completed?

Record the findings, checks performed and reason a repair did not proceed: missing access, unavailable part, unconfirmed diagnosis or a rescheduled job. Add the action required to resume the case.

Can the same template be used on a construction site?

It can be adapted for work on a specific asset. To coordinate multiple contractors and their decisions, use a construction site report. The document has a different purpose and audience.
